Complete the feedback form circulated with the Primary Health & Wellbeing Directory NZ 2024/25 18th Edition and submit it by email or fax as indicated on the form, or
Complete the feedback from available at healthpages.co.nz/feedback, before 8 November 2024.
Winners will be decided on Monday 1 December 2024.
Terms and Conditions:
Entry is open to New Zealand residents only. Officers, employees and contractors to Cervin Limited (NZ company number 424866) (“Cervin”) and their immediate families, or anyone living in their household, are ineligible.
Promotion commences on Wednesday, 18 September 2024 and final entries close at midnight New Zealand Standard Time on Monday, 8 November 2024.
The promotion is a random draw from eligible feedback forms submitted by email, fax or online. There will be one winner.
The Prize is an Apple iPad valued at $699. Prizes are not transferable or exchangeable and cannot be taken as cash.
How to enter: To enter, you must complete the feedback form circulated with the Primary Health & Wellbeing print directory (18th edition) or online at healthpages.co.nz/feedback, including the contact details and all tick box questions. If the print form is completed, it must be returned to Cervin by email or fax using the details provided on the form before midnight on Monday, 8 November 2024.
The winning entry will be randomly drawn from the total pool of eligible entries.
The winner will be notified no later than Friday 20 December 2024 by email to the email address provided on the feedback form. The winner will have until Friday 31 January 2025 to respond to Cervin to claim their prize and provide a delivery address. If the prize is not claimed in that time, another entry will be randomly selected and awarded the prize and the previous winner will forgo all claims to the prize.
Prizes will be shipped by either Cervin or directly by the supplier to the winner. In the event that the prize supplier ships the prize directly to the winner, Cervin must disclose the identity of the supplier to the winner and the winner must authorise Cervin to provide their name and shipping details to that supplier in order for the prize to be delivered. If authorisation is not provided, the winner will forgo all claims to the prize which will then be awarded another randomly selected entry.
Original work: By submitting a feedback form, you agree that Cervin and its agents, licensees, successors, and assigns, shall have the right to print, publish, distribute, and use your submission, including any quotes or statements provided, for news, publicity, information, trade, advertising, public relations, and promotional purposes without any further compensation, notice, review, or consent. You represent and warrant that your submission is your original work and does not violate any third party’s proprietary or intellectual property rights. If your submission infringes upon the intellectual property rights of another, it will be disqualified. You indemnify Cervin from and against any action, proceeding, claims, liability, loss, damage, costs, or expense arising out of any infringement or suspected infringement of any third party’s rights.
This contest is being run by Cervin Limited (NZ company 424866). This contest is in no way sponsored, endorsed or administered by, or associated with Facebook or Instagram or any other social media platform on which it may be promoted (if any).
Limitation of Liability: By entering, You agree to release and hold harmless Cervin and its subsidiaries, affiliates, agents, partners, representatives, agents, successors, assigns, employees, volunteers, officers, and directors from any liability, illness, injury, death, loss, litigation, claim, or damage that may occur, directly or indirectly, whether caused by negligence or not, from: (i) your participation in the contest and your acceptance, possession, use, or misuse of any prize in full or part; (ii) technical failures of any kind, including but not limited to the malfunction of any computer, cable, network, hardware, or software, or other mechanical equipment; (iii) the unavailability or inaccessibility of any transmissions, telephone, or Internet service; (iv) unauthorized human intervention in any part of the entry process or the contest; (v) electronic or human error in the administration of the contest or the processing of entries.
