about TBI Health Care Pathways (ICP-MSK)

TBI Health's Integrated Care Pathways is a fully-funded service providing ACC clients fast access to a specialist-led triage assessment and rehabilitation service (including surgical and non-surgical treatment pathways).

Who is eligible?
We can support patients with:

  • Traumatic Knee injuries including ligament rupture (i.e. ACL), fracture or dislocations and meniscal tears
  • Shoulder fracture, rotator cuff rupture or instability Lumbar disc prolapse with radiculopathy or fracture

The 'Navigator' through the rehabilitation process

  • Triage assessment: Before starting, patients will have an initial assessment with one of our trained team. Our triage assessors will assess their injury, diagnose their current problem and work with you to determine the level of care they require
  • The treatment plan: Following the triage assessment, we will work with your patient to determine which health professionals are best suited to help with your patient's recovery. We will also keep you, the GP, in the loop so everyone is on the same page
  • Return to work: Work is an important part of everyone's life. Research shows that the sooner a patient is back to work safely after an injury, the better their overall recovery is. We may not be able to get everyone back to work right away, but we can usually find strategies to get your patient back to work in a safe way. We also work with your patient's employer to make the transition back to work easier to manage
  • Specialist support: If surgery is the best option for your patient, we can support them through the entire process. From specialist assessment, though surgery and post-operative care to ensure your patient get the best possible outcome
