About Healthcare Rehabilitation

At Healthcare Rehabilitation we understand how important it is to people to maintain control of their rehabilitation after an injury, regain their potential and live life as independently as possible at home and in their community.

We specialise in providing injury-related home support and nursing services that support people on their rehabilitation pathway. Our specialisation means people receive a healthcare service totally focused around the nature of their injury and their everyday needs.

Services can be short term through to long term complex needs, including serious spinal and traumatic brain injury.

Above all, we’re committed to providing each and every person with the best possible rehabilitation service.

Realise Support Service
Our Realise Support service is about providing a specialised home and community support service to people to get on with living life after an injury.
Household management, personal support, social and community integration and child care, are all part of the Realise service but we think its the added extras that make the difference:

*we support people to make informed choices
*we provide support workers that are best suited to a person’s needs
*our support teams are highly trained
*mobile service managers and locally based service managers make it easy to talk face to face with us
*we work collaboratively with other healthcare organisations involved with your support
*we provide a nationwide service so if you travel you continue to receive individualised support

Realise Nursing Service
Our Realise Nursing service is about providing a specialised injury-related nursing service to people in their home. The service is based on ’whole-person thinking’, where we not only treat the symptom or condition but look at ways to minimise the chance of the injury happening again.

*wound management – surgical, traumatic and chronic
medication support
*urinary and bowel assessment and management
*diabeties support and education
