Long Covid / ME-CFS (Chronic Fatigue Syndrome) (also called post-COVID-19 condition or PASC) involves the identification, assessment and management of ongoing or new health problems after COVID-19 infection. It includes confirming the condition using accepted definitions (effects typically beginning within 3 months and lasting at least 2 months, often taken as longer than 12 weeks) and excluding other explanations; providing person-centred care planning, education and follow-up; coordinating care across general practice, allied health and specialist services; and supporting return to work or study. Long Covid care is multidisciplinary and delivered primarily in primary care, with structured pathways to hospital and community providers.
