Challenges: During short consultations, GPs face time limitations which can result in incomplete assessments, referral delays, heightened risk concerns, and increased workload pressure. Our expert Mental Health Nurses are here to alleviate this pressure.
How We Support GPs:
- Time Constraints: We provide comprehensive assessments, freeing your time.
- Incomplete Assessments: Our skilled Mental Health Nurses offer in-depth evaluations and actionable recommendations.
- Referral Delays: Assessments can inform referrals with upfront evaluations, increasing acceptance without delay.
- Risk Issues: We conduct comprehensive risk assessments and ongoing monitoring to help ensure client safety.
- Continuity: We assist with navigation, identify alternatives for unsuccessful referrals, and provide ongoing support.
- Workload: Specialists in assessments/triage (not therapy) help lighten your load.
Value Added to GPs Workflow:
- Customised assessments & treatment strategies
- Optimal triage options for varying needs
- Follow-up & short-term assistance
- Reports & care coordination via HealthLink / Indici
- Thorough assessments & continuous patient support until long-term support is secured
- Guidance with risk / acuity monitoring
Our Workflow:
Perform comprehensive assessments, prioritise triage, create care plans, share assessment details to assist referrals, coordinate via HealthLink / SR, support navigation & track risk / acuity, implement interventions with education, information, advice & guidance. We work with Whānau also. We closely follow BPAC recommendations for assessment and triage.
