Our Workflow:
Perform comprehensive assessments, prioritise triage, create care plans, share assessment details to assist referrals, coordinate via HealthLink / SR, support navigation & track risk / acuity, implement interventions with education, information, advice & guidance. We work with Whānau also. We closely follow BPAC recommendations for assessment and triage.
