about Evolution Care Pathways (ICP-MSK) with Proactive

ICP-MSK is a fully funded ACC musculoskeletal programme. The programme uses an interdisciplinary team who ensure fast access to treatment and rehabilitation for those who have had a significant lower back, knee or shoulder injury.

Who is eligible?
The service is available to anyone who has an accepted ACC covered injury for their knee, shoulder o lower back and may require a team approach including surgery or specialist review as part of their recovery.

How ICP works

Initial call to greet your patient and ask a few questions about their injury
Book your patient in with a physio for an initial assessment
Allocate an ICP Navigator to guide you to stay on track in your recovery
Development of an individualized recovery plan
Early treatment and support based on your patients needs and goals
Support your patient through surgery (if required)
Work with your patient to achieve your recovery goals

Who supports my patient?

Your patient will have a designated person called an ICP Navigator to help support them through their recovery journey. They will help co-ordinate the team that supports your patient. The team may include:

  • Physiotherapist
  • Surgeon
  • Sport Exercise Medicine Specialist
  • Pain management specialists
  • Vocational specialist
  • Psychologist
