about TBI Health Integrated Care Pathways (ICP-MSK) ACC Pathways

is for ACC patients who require customised support to recover from specific knee, shoulder or back injuries.

This fully funded initiative provides fast access to a specialist-led triage assessment and rehabilitation service, including surgical and non-surgical treatment pathways.

We provide:

  • Orthopaedic Specialist support
  • Physiotherapy & rehabilitation
  • Pain management
  • Return to work support

We accept referrals for:

  • Traumatic Knee injuries including ligament rupture, fracture or dislocations and meniscal tears.
  • Shoulder fracture, rotator cuff rupture or instability.
  • Lumbar disc prolapse with radiculopathy or fracture.
  • Post-Traumatic Osteoarthritis of knee or shoulder.
  • Metalware removal from a previous ACC covered fracture of the knee or shoulder.

The rehabilitation process:
Triage assessment: Our team will assess your patients injury and work with you to determine the level of care they require.
The treatment plan: We will determine which health professionals are best suited for your patients recovery. We will keep you, the GP, in the loop.
Return to work: We work with your patient, and their employer to make the transition back to work easier to manage.
Specialist support: If surgery is required, we can support your patient from the initial assessment through to post-operative care.
